These studies were conducted by engineering consultants Bomel Ltd and Loughborough University. Theseinvestigations were completely separate but produced a number of similar findings. Suggestions from the survey included the fact that accident investigation was often superficial and poor supervision and communication were often a significant contributor to accidents.
