520 _aThe Health and Safety Commission (HSC) launches a consultation on its proposal to introduce a compulsory duty for all companies and organisations to investigate reportable workplace accidents which have resulted in serious injury. The proposals would require employers to: investigate all reported incidents to find out how they happened and how they might be prevented in the future; keep a report of the incident and how the conclusions have been taken into account in revising the workplace risk assessment.
